Home Away from Home (ADHC) is a community-based program designed to meet the needs of adults with psychiatric, physical and/or cognitive impairments and chronic health conditions.
Structured, comprehensive and non-residential, this program provides a variety of social, health, rehabilitative and related support services in a safe, culturally diverse setting.
Our goal is to help prevent premature nursing home placement by offering programs and services for participants and respite for family.

What is Unique about Home Away From Home?
The size and layout of Home Away From Home allows for participants to do a variety of things in different settings. Our goal is to have a center that is both diverse in culture and in age. Many adult disabled individuals express a need for a facility to get much needed social supports, while elders need a place in-between a senior center and a long-term residential setting.
We will also contract with many health care professionals/therapists to provide services here in the facility. In addition we will provide transportation to and from medical appointments as needed.
